What is support Coordination ?
Our Support Coordinators are committed to supporting individuals, families and carers to navigate the disability service system by providing advice, information and assistance when decisions need to be made. Our aim is to increase your confidence and understanding of the NDIS and other providers so that you no longer require further assistance.
Once approved by the NDIS, some people are allocated funds to work with Support Coordinators.
As a registered Support Coordination provider, we work to maximise your NDIS plan and connect you to the support you need.
Our aim is to enable the goals as set out in your plan and support you to thrive.
Building Capacity and connecting

Level 1 - Support Connection
- Help you to understand your plan.
- Connect with Supports and Services.
- Identify and create opportunities for you to engage more independently.
- Identify support preferences based on your goals.
- Design a schedule of services which meet your needs and is in line with your expected Plan spend.
- Assist you to navigate and work with multiple providers.
Level 2 - Coordination of Supports
- Design Support Approaches.
- Build Capacity and Resilience in dealing with unexpected or challenging situations.
- Crisis: Planning, Prevention, Mitigation and Action.
- Understanding your personal circumstances and environment including any potential and emerging risks.
- Identify and address any Barriers that could effect you accessing your supports.
- Preparing for unexpected events or situations that might arise.
- Linking you to broader systems of support such as education, justice, child and family protection
